PATIENT DATA LOG FROM MARCH TO JUNE - 2024 03/20/24 Patient Advocate: Respected Sir,                         আমি একজন রোগীর অ্যাডভোকেট। আমার রোগীর জন্ম 30/04/2021 তারিখ, জন্মের সময় তার ওজন ছিল 2.8 কেজি l গত 25/02/24 তার একবেলার জ্বর হয়েছিল তারপর থেকে তার শশীর টা ক্রমশ দূর্বল হয়ে পড়ছে আবার খাওয়া দাওয়া আগের থেকে অনেক বেড়ে গেছে,কিন্তু শাশীরের কোনো উন্নতি নেই জল ও খাচ্ছে প্রচুর একরাতে দুটো ডায়াপার   দিয়েও হচ্ছিল না । এমন কয়েকদিন দেখার পর 03/03/24 তারিখে ডাক্তার দেখানো হয় । ডাক্তার কিছু ঔষধ দেন এবং কিছু টেস্ট করতে দেন ।সেই ঔষধ খেয়েও কিছু কাজ হচ্ছিল না ।এরপর 08/03/24 তারিখে আবার এক ডাক্তার দেখানো হয় , তিনি কিছু টেস্ট করতে দেন  সেখানে ধরা পড়ে রোগীর টাইপ 1 সুগার হয়েছে। ডাক্তার রোগীকে 5 দিন হাসপাতালে ভর্তি রাখেন । 13/03/24* তারিখে তাকে ছেড়ে দেন এবং প্রতিদিন 3+1  ইনসুলিন নিতে বলেন ।এখন এভাবেই রোগী বাড়িতে আছে । রোগী সকল 7.30 তে ঘুম থেকে উঠে দুটো ক্রিম ক্রেকার বিস্কিট খায়, 9.30 am সে এক বাটি ওটস নেয়, 11.30 am একটি ফল (কমলা/মৌসম্বি/পেয়ারা/অ্যাপেল/বেদানা) নেয়, 1.00 pm  ভাত (ডাল,
